I want the LGF to treat the enclosed and all donations I make from the date of this declaration until I notify otherwise as Gift Aid donations. I understand that if this declaration covers future donations, I must notify LGF of any change of name or address and that I can cancel the declaration at any time by notifying LGF. I confirm that I have paid sufficient income or capital gains tax to cover the tax on my donations, and that if my tax circumstances change and future donations no longer qualify for Gift Aid, I will notify LGF.
